  • New genetic variants associated with major adverse cardiovascular events in patients with acute coronary syndromes and treated with clopidogrel and aspirin - Unknown journal (n.d.) · Unknown authors · PubMed 34158603

ABSTRACT: Although a few studies have reported the effects of several polymorphisms on major adverse cardiovascular events (MACE) in patients with acute coronary syndromes (ACS) and those undergoing percutaneous coronary intervention (PCI), these genotypes account for only a small fraction of the variation and evidence is insufficient. This study aims to identify new genetic variants associated with MACE end point during the 18-month follow-up period by a two-stage large-scale sequencing data, including high-depth whole exome sequencing of 168 patients in the discovery cohort and high-depth targeted sequencing of 1793 patients in the replication cohort. We discovered eight new genotypes and their genes associated with MACE in patients with ACS, including MYOM2 (rs17064642), WDR24 (rs116401

  • GWAS of allometric body-shape indices in UK Biobank identifies loci suggesting associations with morphogenesis, organogenesis, adrenal cell renewal and cancer - Unknown journal (n.d.) · Unknown authors · PubMed 34021172

ABSTRACT: Genetic studies have examined body-shape measures adjusted for body mass index (BMI), while allometric indices are additionally adjusted for height. We performed the first genome-wide association study of A Body Shape Index (ABSI), Hip Index (HI) and the new Waist-to-Hip Index and compared these with traditional indices, using data from the UK Biobank Resource for 219,872 women and 186,825 men with white British ancestry and Bayesian linear mixed-models (BOLT-LMM). One to two thirds of the loci identified for allometric body-shape indices were novel. Most prominent was rs72959041 variant in RSPO3 gene, expressed in visceral adipose tissue and regulating adrenal cell renewal. Highly ranked were genes related to morphogenesis and organogenesis, previously additionally linked to can

Discuss with your doctor

  • clopidogrel response testing and alternative antiplatelet therapy High

rs11640115 G allele carriers show 2.08-fold increased MACE risk with clopidogrel-aspirin, indicating reduced thienopyridine responsiveness via WDR90 cardiac expression dysregulation

Request thienopyridine response testing; discuss prasugrel 5-10mg daily or ticagrelor 60mg twice daily as alternatives

Drug interactions

  • antiplatelet therapy adherence and dosing verification High

Baseline clopidogrel resistance requires strict compliance with prescribed dosing to optimize antiplatelet protection

Verify 75mg daily clopidogrel or prasugrel/ticagrelor dosing if switched; assess adherence at each follow-up visit

Screening

  • cardiac imaging and event surveillance post-ACS High

G allele carriers demonstrate 2.08-fold elevated 18-month MACE risk; enhanced monitoring enables earlier detection of recurrent cardiac events

Baseline transthoracic echocardiogram and ECG, then repeat at 3, 6, 12 months post-PCI
